The “sefer” the Rambam speaks about is not a sefer torah at all. It is a codex, which is a primitive form of bound book, not a scroll. And unlike a sefer Torah, it is written with nekudos and trop and includes Neviyim & Kesuvim. This work, known as the “Aleppo Codex”, still exists today in a museum, though about 40% of the text is missing, preumably stolen. The missing portion includes nearly the entire 5 chumashim.
