Shouldn’t someone be concerned about they reflect to others and Hashem? Rabbi Yochanan ben Zakkai told his students on his deathbed that their fear of heaven should be upon them like the fear of flesh and blood. A person needs a conscious.

This video was only addressing the issue of how to cure social anxiety. The example from R.Yochanan be Zakkai was not said with regards to people who suffer social anxiety, but rather, was advice about how to have yiras shamayim and do the Mitzvos properly.
