There are many letters of the Rebbe where he writes that when adding a name, to do it as the first name (i.e. Chaim…. Ben…). I think the Rebbe even asked someone who added the name at the end to add another at the front. Stile, when HRHG Moshe Rubin O”H was sick R”L, I remember R’ Volf and R’ Gerlitzky added Chaim as the second name (after his original name) so there must be mekoros the other way too.
